Patient Care Coordinator Job in Grand Island, NY
Trilogy Wellness is hiring for a Patient Care Coordinator job in Grand Island, NY, and we’re looking for someone who enjoys helping people while keeping every part of the patient experience organized and running smoothly. As the first point of contact for many of our patients, you’ll play an important role in creating a welcoming environment, coordinating appointments, and supporting our clinical team. If you enjoy balancing customer service with administrative responsibilities and want to build your career in healthcare, we’d love to meet you.
Help Patients Feel Supported From Their First Call
At Trilogy Wellness, we understand that exceptional care begins long before a patient enters the treatment room. Every phone call answered, every appointment scheduled, and every insurance authorization completed helps patients begin their recovery with confidence. As a Patient Care Coordinator, you’ll be an essential member of our team, ensuring patients receive timely service and clear communication throughout their care journey.
Your day will include scheduling new and returning patients, registering individuals accurately within our electronic medical record system, verifying insurance benefits, obtaining authorizations, and maintaining organized documentation. You’ll also work closely with our therapists to coordinate schedules, resolve appointment conflicts, and help maintain an efficient clinic schedule while supporting a cancellation rate of less than 10 percent.
This position is ideal for someone who enjoys working directly with people while managing detailed administrative responsibilities. You’ll communicate with insurance companies, assist patients with benefit verification, collect payments accurately, manage referrals and Plans of Care, and maintain complete patient records within the EMR. Every interaction contributes to a positive patient experience and helps our clinical team stay focused on delivering outstanding care.
We’re looking for someone who communicates with professionalism, stays organized under changing priorities, and enjoys being part of a collaborative healthcare environment. If you’re motivated by helping others and take pride in accuracy, efficiency, and outstanding customer service, you’ll find meaningful opportunities to grow while making a difference for every patient who walks through our doors.
